WL - UPC Issues & SKU Requisition Forms

Overview

To allow us to better assist you in a timelier manner please make sure to use the proper form when providing the information to the appropriate party at the SSC.


UPC Issues Form

-  To report UPC issues, please use the UPC Issues Form.  This form is to be used when you run across an item that is not scanning or scanning incorrectly.  Please complete it with as much detail as possible.  When complete, open a Zoho ticket with the correct Merchandising Team and attach the UPC Issues Form to the ticket. We understand that it is not always easy to tell why the item will not scan but providing all of the pertinent information up front will allow us to assist you in a timelier manner.  Screenshots will not be accepted as they are not always legible for the recipient.

SKU Requisition Form

-  When you need to have SKUs created prior to placing an order with a vendor so that there are no issues upon receipt of the order, please complete the SKU Requisition Form.  SKUs cannot be created without all pertinent information being provided.  This includes vendor #, UPC, Model/Mfg#, SKU Description, OM, Cost per Each and Retail per Each.  When complete, open a Zoho ticket with the correct Merchandising Team and attach the SKU Requisition Form.  Providing the information in Excel format allows for quicker SKU creation.  The 2024 SKU Requisition Form found at the link below contains two tabs – one specific to product vendors and the other to plant vendors.  Make sure you are using the appropriate tab as additional information is required to set-up nursery items.   Screenshots will not be accepted as they are not always legible for the recipient.

Both of these forms can be found in the Forms folder within the Merchandising Toolbox on the Store Portal.  We ask that you discontinue use of any other forms that you may have saved for these purposes.  Always access the form via the Store Portal in case there are any changes made to the form.  And make sure to send as an attachment to the correct Merchandising Team in Zoho.  Screenshots will not be accepted as they are not always legible for the recipient.
